Team manager contact information.
Coaches names.
Player Names and Numbers.
At least two cell phone numbers to contact team while traveling.
USA Hockey Team # or C.A.H.A. Team # (Canadian Teams).

Travel teams requiring hotel accommodations are required to reserve rooms at the same time they complete an application. A single team contact must reserve a block of rooms and complete the application. The team's contact must use the same e-mail address for the hotel booking and the application's team contact. Please make note of the Hotel GROUP CODE as it will be needed on the application.

Hotels have been pre-selected for each team level, so that teams will be booked near the rinks they will be playing at.
