Checklist of items you will need to register.
 

• Printer to print registration confirmation page.
• USA Hockey team registration number.
  (Canadian teams must register as travel teams)
• Complete Roster and Jersey numbers. (8-20 players)
• Two cell phone numbers to contact team while traveling.
• All contact information for team registrar.
• Name of coaches and assistant coaches and team manager.
• Teams website. (Not mandatory)

To register:

Complete our Local team registration form.

• Print your confirmation page.

• Mail payment : Your check for $ 1400.00 US (1300.00 US for Canadian Teams) 
  must be received along with your confirmation page for application to be valid,

• All checks must be received within 5 working days.

• Checks payable to "Connecticut Polar Bears"
