27th Annual Polar Bears Tournament registration page
       (Local teams Not requiring Hotel reservations)

Checklist of items you will need to register.
 

  • Printer to print registration confirmation page.
  • USA Hockey team registration number.
    (Canadian teams must register as travel teams)
  • Complete Roster and Jersey numbers. (8-20 players), U-14 through U19, players current grade level (7th,  8th, FR, SO, JR, SR.) U12 and U10 teams do not need to enter their current grade level.
  • Two cell phone numbers to contact team while traveling.
  • All contact information for team registrar.
  • Name of coaches and assistant coaches and team manager. All coaches phone numbers.
  • Teams website. (Not mandatory)

To register:

Complete our Local team registration form.

  • Print your confirmation page.

  • Mail payment : Your check for $1450.00 US.
    must be received along with your confirmation page for application to be valid,

  • All checks must be received within 5 working days.

  • Checks payable to "Connecticut Polar Bears"
